These Terms and Conditions ("Terms") govern access to and use of the R+ Radiology Information System (the "Service") by a licensed healthcare provider organization ("Clinic") and its authorized staff. By signing in to the Service, staff confirm they are an authorized user acting on behalf of the Clinic.
The Service does not offer public self-registration. Staff accounts are created and managed exclusively by the Clinic's administrator, who is responsible for assigning appropriate roles and permissions and for deactivating accounts when access is no longer required.
The Service may only be used for legitimate clinical, administrative, and operational purposes of the Clinic. Users must not share login credentials, attempt to access records outside their authorized role, or use the Service in a manner that violates applicable healthcare privacy or data protection laws.
Patients access imaging results only through a unique, time-limited link and credentials issued by the Clinic. Patients are responsible for keeping their access code and password confidential. Links expire automatically and cannot be renewed without the Clinic issuing a new one.
The Service is a workflow and information management tool. It does not provide medical advice or diagnosis, and it does not replace the professional clinical judgment of qualified healthcare providers. The Clinic remains solely responsible for the accuracy of clinical data entered and for all diagnostic and treatment decisions.
While we aim to keep the Service available and secure, it is provided on an "as is" and "as available" basis without warranties of any kind, express or implied, to the maximum extent permitted by law.
To the maximum extent permitted by law, [Company Legal Name] shall not be liable for any indirect, incidental, or consequential damages arising from use of the Service. Nothing in these Terms limits liability that cannot be excluded under applicable law.
Access may be suspended or terminated if these Terms are violated, if required by license enforcement, or upon termination of the underlying agreement between the Clinic and [Company Legal Name].
These Terms are governed by the laws of [Jurisdiction], without regard to conflict-of-law principles.
Questions about these Terms can be directed to [legal@example.com].
